(2) Photocopy of Parent’s Military ID Card.
(3) Letter of Identification from Parent’s Unit.
(4) Birth Certificate or Sworn Declaration of Age.
(5) Part 2/3 Orders Publishing candidates’ birth.
(6) Children of discharged personnel are to present photocopy of parents discharged certificates or letter of identification from a military unit certifying that he/she is a bonafide child of the discharged personnel
(7) Primary school testimonial.
(8) Writing Materials and a file jacket.
b. Civilians’ Children:
(1) Entrance Examination Photo Card.
(2) Birth Certificate or Sworn Declaration of Age.
(3) Primary school testimonial.
(4) Writing materials and a file Jacket.
